If you would like to refer a child for grant consideration from this special fund, please download and complete the forms included below. Submit them electronically to Tracy Taggart, Director of Operations & Grants. A printed version will be accepted if the applicant is not able to submit it electronically. The Community Foundation will contact the family for additional information.

NOTE: The Community Foundation respects the privacy of all children and families we serve. All medical information is confidential and is not disclosed with outside parties unless we have the consent of the child's parent(s) or guardian(s).
